Icici bank-specific port implementations (csv table path).

Faithful port of domain/extraction/banks/icici/csv.py: shares all table logic with xlsx via the shared TableParserMixin (default icici aliases).

IciciCsvExtractionPipeline pins the reader; ordering/balance policy stays in the shared tabular txn_dicts stage.
